 Choose the system that you think is right for you. Think about how many rooms will have DISH Network and if you want a DVR, an HD system, or both. When you speak with an installation professional or service representative they will confirm your equipment preferences. The receiver and programming choices you make on this order form do not need to be your final choices. Room selections needing multiple receivers can result in additional monthly fees.

3. AGREEMENT

With 24 Month agreement, $99 one time activation fee will be waived.
HD Free for Life requires 24 month agreement and AutoPay with Paperless billing.
*If qualifying service is terminated prior to end of 24 months, a cancellation fee equal to the lesser of $420 or $17.50 per month for each cancelled month of service will apply.
